This turnaround takes the basic B7 shape and adds in just enough notes around it to create a two-bar melodic phrase that spells out the sound of the V before resolving to I, all while keeping the alternating thumb going in the bass. Speaking of which, this lick calls for rocking your middle finger back and forth from the fifth to the sixth string. While it may take some getting used to, the moving of your finger back and forth from string to string is a big part of the sound of a lick like this: if you actually had a way to hold down both notes simultaneously, the two bass notes would ring out and overlap, and things would quickly get too muddy on the low end.
